Check your documents

Warning We will not look at your application until you supply the required documents. Your evidence must show how your condition affects your mobility or your ability to make journeys.

When you apply for a Blue Badge, you must provide several documents that support your application. You will save yourself time if you make sure you have the documents available before you start your application.

Documents you must provide

  • Proof of identity
  • Proof of address
  • Passport photo of the badge-holder
  • Supporting evidence

Supporting evidence

You must provide supporting evidence that shows how your condition affects your mobility or your ability to make journeys. Check the supporting evidence you need to provide with a Blue Badge application before you start your application.

Proof of identity

One of the following (for an adult).

  • Valid driving licence
  • Valid passport
  • Birth/adoption certificate
  • Marriage/divorce certificate

One of the following (for a child).

  • Valid passport
  • Birth/adoption certificate

Proof of address

One of the following (for an adult).

  • A Council Tax document for the current tax year
  • A utility bill or financial statement dated within the last three months
  • A fully signed, termed tenancy agreement - we only require the pages that show the tenant names, property address, tenancy term dates and the signatures
  • An official letter from an organisation such as HRMC, NHS, benefits agency, local council – dated within the last three months
  • A payslip dated within the last three months
  • A valid driving licence

One of the following (for a child).

  • A letter from the child's school – dated within the last three months
  • An official letter from an organisation such as HRMC, NHS, benefits agency, local council – dated within the last three months
  • A letter from a hospital, GP or appointment - dated within the last three months
  • Letter to the parents but about the child

Passport photo of the badge-holder

Acceptable photograph - the photograph must be:

  • a close-up photo of the head and shoulders of the badge-holder
  • in colour
  • taken within the last six months
  • with no one else visible.

Getting your medical summary during the Coronavirus (COVID-19) outbreak

Do not visit your doctor's surgery to get your medical summary.

If you have access to your patient records via one of the online services, such as Patient Access or Evergreen Life, you can get your medical summary. This service is free. If you contact your surgery, they may provide you with a copy of your medical summary. You will not be charged for this service.

Applying under 'non-visible (hidden) disability'

If you are applying under the non-visible (hidden) disability criterion, you can ask your healthcare professional to complete an expert assessment form as evidence of your condition. Find out more about expert assessors for Blue Badge applications.

How to get further medical information

To help us assess your eligibility for a Blue Badge, we will require information regarding your medical history. The medical information needs to show how your condition affects your mobility. You may have letters from consultants or specialists at home due to ongoing appointments that can be used as evidence to support your application.

Alternatively, with online patient access, you can download a copy of your medical history directly to your laptop/computer. If you cannot access your medical summary online, please contact your GP’s Reception team. There is no need for you to contact your GP directly.

Payment

If your application is approved, you will be asked to pay a £10 printing fee. An advisor will call you to take a telephone payment. Payment is also accepted in the form of a cheque or postal order made payable to Oxfordshire County Council. Please do not attempt to make a payment until you have been informed your application has been approved.
