What happened at your needs assessment
At your needs assessment, we talked together about:
- what is and who are important to you
- what is stopping you from doing what you would like to do or seeing who you would like to see
- how you want your life to be
- what you are having difficulty with
- what support you have and what support might enable you to live more independently
- we will also talk to your partner, relatives and friends if you want us to
During the assessment, we built a complete picture of you, what's important to you, and the different aspects of your life.

If you are eligible for support from us
After your needs assessment, if you are eligible for our support, we will work with you to develop a support plan.
Your support plan
Your support plan will outline what you want to achieve and how you want your needs to be met. To ensure that you get the right support, we may talk to other health or social care professionals, but we'll always ask your permission before we do so. We know things change, so your support plan will be reviewed regularly.
We will then start working closely with you to put in place the services that will support you.
If you are not eligible for support from us
If you're not eligible for our support, there is a lot of local help that you can get to help with your care.
