To make your report, you will need the following:

  • Details of the person you are concerned about
  • Date and details of the incident/concern
  • Any injuries or doctors notes
  • Details of the alleged perpetrator (if known)

Reporting more than one person

If you wish to report a concern about another person, you will need to submit a new form for each person.

The Oxfordshire Safeguarding Adults Board

The Oxfordshire Safeguarding Adults Board (OSAB) has the statutory responsibility for bringing together organisations in Oxfordshire to decide how we can work together at a strategic level to best safeguard adults at risk of abuse and neglect in our community.

They also have a responsibility for conducting reviews when an adult with needs for care and support in Oxfordshire dies or is seriously injured as a result of abuse or neglect and there are concerns about how organisations worked together to protect the person.
