Reviewing an EHC (Education Health and Care) plan

How an EHC plan is reviewed and how you will be involved.

Termly review

Your child or young person’s setting or school should invite you to meet with them three times a year to review the progress your child or young person is making. One of these meetings will be the Annual Review.

Annual review

The annual review of an Education Health and Care plan looks at the outcomes that were decided previously and the provision set out in the plan. The child or young person is at the centre of this process and so is involved in discussion and decisions along with you as parents and the professionals who work with your child. Everyone at the meeting will talk about the progress made and agree new targets for the coming year. Any changes to the plan are discussed, including whether an EHC plan is still needed.

When will the annual review be held?

The first annual review must be held within 12 months of the date when the EHC plan was issued and after that within 12 months of the previous review.

The local authority will write to your child’s headteacher or head of setting two terms before the review is due to ask them to arrange the review of your child’s EHC plan.

What will happen?

  • About eight weeks before the review meeting the school will write and invite you to the meeting and send a form for you to record your views (docx format, 219Kb) of the progress your child is making and wishes.
  • You should complete and return this by three weeks before the meeting.  It will be sent to everyone invited. Your child will also be asked for his or her views (docx format, 224Kb).
  • About two weeks before the meeting you will receive copies of the reports that people who support your child have written for the review.  This will include people from the setting or school and also from outside agencies, for example an educational psychologist or speech and language therapist.
  • At the meeting you will be invited to talk about your child’s strengths, their progress in and out of school and what new targets they could work towards over the coming year.  If they have a personal budget this will be discussed too.  Any suggested changes to the plan will be written down and be sent by the school to your child’s SEN Officer within two weeks of the meeting.
  • At the end of the meeting you will be given an evaluation form to complete online.  This will help us to improve the process. There is also an evaluation form for your child to complete.
  • When your child’s SEN Officer receives the completed record of the meeting they will decide if any changes need to be made to the plan and let you know within four weeks of the meeting.
  • If changes are to be made you will receive an amended copy of the EHC plan within eight weeks of the meeting and you have 15 days to comment on it. The SEN Officer will then decide whether to continue to make amendments and will issue an amended plan, or let you know why not.
