We will not look at your application until you supply the required documents. 35 per cent of applications are delayed because of insufficient information or missing documents. Your evidence must show how your condition affects your mobility or your ability to make journeys.

When you apply for a Blue Badge, you must provide several documents that support your application. You will save yourself time if you make sure you have the documents available before you start your application.

Documents you must provide

  • Proof of identity
  • Proof of address
  • Passport photo of the badge-holder
  • Supporting evidence

Supporting evidence

You must provide supporting evidence that shows how your condition affects your mobility or your ability to make journeys. Check the supporting evidence you need to provide with a Blue Badge application before you start your application.

Proof of identity

One of the following (for an adult).

  • Valid driving licence
  • Valid passport
  • Birth/adoption certificate
  • Marriage/divorce certificate

One of the following (for a child).

  • Valid passport
  • Birth/adoption certificate

Proof of address

One of the following (for an adult).

  • A Council Tax document for the current tax year
  • A utility bill or financial statement dated within the last three months
  • A fully signed, termed tenancy agreement - we only require the pages that show the tenant names, property address, tenancy term dates and the signatures
  • An official letter from an organisation such as HRMC, NHS, benefits agency, local council – dated within the last three months
  • A payslip dated within the last three months
  • A valid driving licence

One of the following (for a child).

  • A letter from the child's school – dated within the last three months
  • An official letter from an organisation such as HRMC, NHS, benefits agency, local council – dated within the last three months
  • A letter from a hospital, GP or appointment - dated within the last three months
  • Letter to the parents but about the child

Passport photo of the badge-holder

Acceptable photograph - the photograph must be:

  • a close-up photo of the head and shoulders of the badge-holder
  • in colour
  • taken within the last six months
  • with no one else visible.

Applying under 'non-visible (hidden) disability'

How to get further medical information

We require information about your medical history to help us assess your Blue Badge eligibility. The medical information must show how your condition affects your mobility. You can use letters from consultants or specialists as evidence to support your application.

With an NHS login, you can access your medical records via the NHS website or the smartphone app. You may be able to download copies of documents to use as evidence in your application.

Using the NHS app to access your medical information

With the free app, you can:

Contact your GP's reception team if you cannot access your medical summary online. There is no need for you to contact your GP.

Some surgeries may use a different app, but you can still use the NHS app for the same actions.

Payment

You will be asked to pay a £10 printing fee if your application is approved. An advisor will call you to take a telephone payment. Payment is also accepted as a cheque or postal order made payable to Oxfordshire County Council. Please do not attempt to pay until you have been informed your application has been approved.
