Documents and forms for childminders

Help to support childminders in meeting the learning and development requirements.

Ofsted has regard to the Early Years Foundation Stage (EYFS) in carrying out inspections, and reports on the quality and standards of provision.  The framework is mandatory for all early years providers on the Early years Register.

The following documents will support you in meeting the learning and development requirements and the safeguarding and welfare requirements of the EYFS.

Early Years and Childcare Register

CR1 form is for the Childcare Register only (on the voluntary and/or compulsory part) CR2 form is for every person other than the applicant who requires checking, it is the declaration and consent form to carry out checks and decide their suitability

Early years online self-evaluation form (SEF) and guidance for providers delivering the Early Years Foundation Stage Ref: 080104 - recording and showing the inspector that you have improved since your last inspection

Early Years Foundation Stage. It is important that you refer to this document when self-evaluating your provision.

The self-evaluation form helps you in recording and showing the inspector that you have improved since your last inspection. Ofsted encourages you to complete the SEF online.

Ofsted security token

You should have received a unique password from Ofsted called an Ofsted Security Token (OST) and the guidelines on how to complete the online form. If you have not received these, please contact Ofsted on 0300 123 1231, quoting your Ofsted Unique Reference Number.

Alternatively, you can complete the SEF by hand, and post it to Ofsted. The SEF and Guidance documents are on the Ofsted website.

Good practice

Good practice resource - Telling tales: using storyboards and homemade books to promote learning in the early years - A childminder records children’s learning with cheap, easily made storyboards. These create lasting memory books which are used to record children’s interests and achievements and are greatly enjoyed by children and their parents and carers.
